Intervention Planning
Each client will have an Intervention Plan outlining their goals and focus areas for the next period until the review date. This can be done formally or informally with the client's advocate/trusted decision maker. Client is to be included as much as reasonably possible.
The Intervention Plan will be developed in consultation with the client and pertinent others who may include: guardian, family member, appointed decision maker, advocate and Provider. This will be reviewed regularly with the parent/caregiver.
It is part of NeurodiversOTy’s process to have these check ins at least once per school term or 4 times per year to ensure they are updated appropriately and conducive to a client's continual progress. This can be subject to change based on the feedback and engagement of the client’s support network throughout the process of intervention. Due the everchanging nature of disability, the frequency of this can be adapted in collaboration with the client/caregiver and your child's principal therapist.
The client’s parent/caregiver can request a review meeting or an education meeting at any time and can organise a time with their OT based on availability of appointment times.
